Clinical fellowship versus observership
The name alone is not enough. Two programmes described as a fellowship may offer very different access to patients, operating rooms, research time and institutional appointments.
Educational guidance, not legal, immigration, licensing or admissions advice.Clinical fellowship
Usually involves a formal clinical appointment or defined supervised clinical role. The exact scope still depends on registration, credentialing, indemnity and local policy.
- Look for explicit patient-care and operative responsibilities
- Check registration and licensing requirements
- Confirm salary, stipend or fees separately
Observership
Primarily permits observation of clinics, operating lists, meetings and teaching. It should not be interpreted as permission to examine, consent, prescribe, operate or provide independent care.
Travelling fellowship
Often combines short visits to several hosts with networking and education. Funding may cover travel rather than employment, and clinical access can vary by centre.
How OrthoFellow classifies listings
We preserve the provider’s exact programme type, then organise it by the role supported by the source. Missing privilege information remains labelled as unpublished.
Sources checked and applicant checklist
The comparison is role-based. A label alone does not override regulator, hospital or host restrictions on patient contact.
Before you act
- Ask what the fellow may observe, assist with or perform under supervision.
- Confirm whether registration, hospital credentialing and indemnity are required.
- Compare the promised role, supervision, duration and outputs in writing.
- Treat a programme title or host listing as insufficient evidence of clinical privileges.

Frequently asked questions
Can an observership become hands-on after arrival?
Do not assume so. Any change requires explicit host approval and may require registration, credentialing and insurance. Obtain written confirmation before planning around clinical participation.
Is a travelling fellowship employment?
Usually not, unless the provider clearly states an employment appointment. A grant for travel or accommodation is different from a salary.
