Clarify the clinical role
Ask what fellows may observe, assist, perform under supervision or manage independently—and which activities require registration or additional credentialing.
- How is operative responsibility allocated?

Understand the case mix
Request the distribution that matters to your objective: primary versus revision, elective versus trauma, adult versus paediatric, or specific procedures. Total departmental volume does not equal fellow exposure.
Assess supervision and progression
Ask who supervises the fellow day to day, how autonomy changes, how many learners share cases, and what happens when the expected case mix is unavailable.
Examine research and teaching expectations
Clarify protected time, available datasets, methods support, authorship expectations, meetings, teaching duties and whether projects can realistically finish within the appointment.
Confirm logistics and financial terms
Reconfirm dates, salary or stipend, fees, leave, registration costs, insurance, visa sponsorship, relocation and accommodation. Ask which items are confirmed in writing and which remain conditional.
Close with outcome-focused questions
Ask what previous fellows typically achieved, how the programme responds when objectives are not being met, and what preparation would make the selected candidate effective from the first month.
