Orthopaedic Fellowships in the UK: Routes & Visas
Most UK orthopaedic fellowships are salaried NHS posts, usually titled clinical fellow or senior clinical fellow, that a trust advertises and fills itself. A smaller group are nationally coordinated: the JCST Interface Fellowships, and fellowships quality-assured under the RCS England Senior Clinical Fellowship Scheme. Separately, the BOA and the specialist societies award travelling fellowships and bursaries, which are grants rather than jobs. International applicants also need GMC registration with a licence to practise and, usually, a Health and Care Worker visa. The Medical Training Initiative (MTI) closed to new applicants at 12 noon on 31 March 2026, so the remaining routes matter more than they did. There is no national match for orthopaedic fellowships in the UK. Most posts are employed jobs in an NHS trust or health board, advertised on NHS Jobs or a trust's own recruitment site and filled by local interview. Two national frameworks add structure on top of this. The Joint Committee on Surgical Training (JCST) runs Interface Fellowships for areas of practice that span more than one specialty. RCS England, working with the Surgical Specialty Associations, quality-assures individual units' programmes through its Senior Clinical Fellowship Scheme (SCFS). Awards from the BOA and the specialist societies are different again: they fund a period of learning, often a visit away from your base, and do not employ you.
- JCST Interface Fellowships: overseen by Training Interface Groups (TIGs) in seven areas. These are cleft lip and palate, hand surgery, head and neck surgical oncology, major trauma, Mohs surgery, oncoplastic breast surgery and spinal surgery.
- RCS England SCFS: aimed at senior trainees (ST6 to ST8) in the UK and internationally. Posts last between six months and two years. A programme's approval lasts three years, and you apply for the post itself through the NHS trust's jobs website.
- Trust clinical fellow posts: advertised individually, mostly on NHS Jobs. Each trust sets its own pay, duties and on-call commitment.
- Society and BOA fellowships: competitive grants or travelling awards, usually open only to members.
JCST Interface (TIG) fellowships: spine, hand and major trauma
Three of the JCST Interface Fellowships are relevant to trauma and orthopaedic surgeons. The Spinal Surgery fellowship aims to produce surgeons who can manage the full range of elective and emergency spinal conditions, and it is open to surgeons from neurosurgery and trauma and orthopaedic surgery. The Hand Surgery fellowship lists approved units across England, Scotland and Wales. The Major Trauma fellowship is open to surgeons from five specialties, including trauma and orthopaedic surgery, and offers two pathways: the resuscitative surgeon and the major trauma consultant. On each fellowship page, JCST notes that not all units will be available or accepting a fellow in a given year.
- Spinal Surgery: 10 approved units are listed, including Leeds, the Walton Centre, Bristol, Oxford, Birmingham, Preston, Sheffield, UCLH with the RNOH, Cardiff and Stoke. Recruitment information is on the Severn Postgraduate Medical Education website.
- Hand Surgery: approved units are listed from Cambridge to Wrightington, and JCST also directs applicants to Severn for how to apply. BSSH lists Training Interface Group fellowships among the fellowships it is affiliated with.
- Major Trauma: on 1 October 2026 JCST announced 10 posts across 6 training centres, with recruitment coordinated by Barts Health NHS Trust and a closing date of 12 October 2026.

BOA, BOTA and specialist society fellowships
The BOA runs an annual round of travelling fellowships for its members. For 2026 they were open to Home Associates, Post-CCT members and SAS Surgeon members whose subscriptions were up to date. Post-CCT members were eligible only if they had not taken up, or been appointed to, a substantive consultant post by the closing date. The BOA states that all its fellowships are for UK members only, and it advises overseas surgeons to contact their own national association. The BOA's specialist society page links to fellowship schemes run by BESS, BHS, BOFAS, BSCOS, BSSH, BOSTAA, BLRS and others.
- BOA 2026 awards: Worldwide fellowships of £1,500 (3 available), £3,000 (2) and £10,000 (1); a UK fellowship of £3,000 (1); an SAS Education Fellowship of £3,000; the Heraeus-funded Andrew Sprowson infection fellowship of £1,500; and a Soli Lam Spinal fellowship of up to £15,000.
- BOA rules: a fellowship can last up to 12 months from the date of travel, must take place outside your home trust and cannot be used to attend conferences.
- BOTA: according to the BOA's specialist society page, BOTA awards Senior Orthopaedic Fellowship Grants together with the British Indian Orthopaedic Society (BIOS).
- BESS: tries to award at least five fellowships a year at its annual meeting, after interview. It also keeps a directory of UK shoulder and elbow fellowships and has a standards framework agreed with RCS England.
- BHS: for 2026/27 it offers travelling fellowships, a £4,000 bursary and two £2,500 grants for members. The bursary and grant deadline is 31 October 2026, with interviews at the March 2027 annual meeting.
- BSSH and BSCOS: BSSH runs the Stack, Research, Overseas and International Visiting fellowships. BSCOS lists paediatric fellowship posts in the UK and abroad but says it is not responsible for those posts.
- BASK: this guide could not open the BASK fellowships page, so check the BASK website directly.
Clinical fellow posts on NHS Jobs and local pay
Most orthopaedic fellowships in the UK are filled through ordinary NHS recruitment. The RCS England register states that candidates for its approved T&O programmes must apply through the NHS trust's jobs website. Several BSCOS-listed paediatric units say they advertise on NHS Jobs and will not accept applications made any other way. Pay for these posts is set locally. The NHS Employers 2026/27 pay circular says employers may pay doctors on local contracts, including clinical fellows, on scales that mirror those for doctors in training. It adds that there is no requirement to use those values, because local terms and conditions are a matter for local determination.
- On 8 October 2026, a search of NHS Jobs for 'orthopaedic fellow' showed, for example, a sarcoma and hip arthroplasty clinical fellow post at £76,582 a year and a foot and ankle senior clinical fellow post at £67,325 to £76,582 a year. These are snapshots of single adverts, not typical rates.
- Before applying, check the job description for the pay scale or code, banding, on-call commitment, the length of the post and whether any operating is supervised.
- Some units advertise once a year. One BSCOS-listed unit, for example, advertises its 12-month post in November, interviews in mid-December and starts the post the following August.
GMC registration for international graduates
To work as a doctor in the NHS you need GMC registration with a licence to practise. For international graduates, one main route to full registration is sponsorship. To use it, you must have been selected for postgraduate training by a UK medical royal college or faculty, or be sponsored by a body on the GMC's approved list. You must also have practised medicine for three of the last five years, including ten of the most recent 12 months. The sponsor assesses you against its GMC-approved scheme criteria and issues a Sponsorship Registration Certificate (SRC). The GMC will not accept a UK visa certificate of sponsorship in its place. The GMC's sponsorship guide does not list a PLAB pass among its eligibility criteria.
- Approved sponsors relevant to surgery include the Royal College of Surgeons of England, the Royal College of Surgeons of Edinburgh and the Royal College of Physicians and Surgeons of Glasgow. The list also includes trust schemes, such as Sheffield Children's 'Trauma and Orthopaedic International Fellows'.
- RCS England GMC Sponsorship Scheme: you need a formal job offer from an NHS trust or health board at ST3 or above. You also need at least three years of full-time clinical practice since your primary medical qualification, including the most recent 12 months, with gaps of no more than 5 weeks. You must not have failed any part of PLAB. Assessment can take up to 4 months.
- RCSEd International Postgraduate Deanery (IPD): this replaced the IMGSS and RISFP programmes. It sponsors international medical graduates for GMC registration. The fee is £775 for Route 1 and £1,000 for Route 2. Applicants must have been in clinical surgical practice for 3 of the last 5 years.
- MTI closure: NHS Employers confirms that the Academy of Medical Royal Colleges closed the MTI to new applicants from noon on 31 March 2026. Applications already in progress that involved a royal college had a grace period until 30 June 2026, and doctors already on the scheme continue until their posts end.
- RCS England describes its International Surgical Training Programme (ISTP) as an MTI scheme. RCSEd says that, with the MTI closed, it will work with IPD applicants to identify a suitable alternative visa.
English language requirements
The GMC needs evidence of your knowledge of English before it grants registration with a licence to practise. If you apply through sponsorship, you must show this with an IELTS (Academic) or OET (Medicine) certificate awarded within the last two years. The GMC cannot extend the validity of a certificate for sponsored applicants.
- IELTS (Academic, including UKVI): at least 7.0 in each area and at least 7.5 overall, achieved in the same test at your most recent sitting.
- OET (Medicine): at least grade B in speaking, listening, reading and writing, achieved in the same test at your most recent sitting.
- Validity: both certificates are valid for two years and must still be valid when the GMC approves your application.
- Visa: for the Health and Care Worker visa, GOV.UK says doctors do not need to prove their English again if they have already passed an English assessment accepted by the relevant regulated professional body.
Health and Care Worker visa: fees and salary rules
International fellows employed by the NHS usually come on the Health and Care Worker visa. You need a job offer from an approved employer, such as the NHS, and a certificate of sponsorship from that employer. Generalist and specialist medical practitioners are eligible occupations (codes 2211 and 2212). You must apply within 3 months of receiving the certificate of sponsorship. The visa can last up to 5 years before you need to extend it, and you will usually get a decision within 3 weeks.
- Application fee: £324 per person for up to 3 years, or £628 per person for more than 3 years. The fee is the same whether you apply from inside or outside the UK.
- Immigration health surcharge: you, your partner and your children do not pay it.
- Maintenance funds: you will usually need at least £1,270, held for 28 days in a row, unless you are exempt or your employer certifies maintenance on your certificate of sponsorship.
- Salary: you must be paid at least £25,000 or the going rate for the job, whichever is higher. For doctors, GOV.UK points to its national pay scales table.
- England going rates in that table (last updated 22 July 2025): specialty registrar at CT/ST3 and above, £61,825; specialty doctor, £59,175; specialist doctor, £96,990. Ask your employer which grade your fellowship is mapped to.
NHS pay in 2026/27 (England)
Pay and Conditions Circular (M&D) 1/2026 R2, dated 24 July 2026, sets national medical pay in England from 1 April 2026. It applies a 3.5% increase for resident doctors, SAS doctors and consultants. Following the government–BMA agreement, it also restructures the resident doctor nodal pay points from five to ten. Fellowship posts on local contracts may be paid on these scales or on a locally set rate.
- Specialty doctor (2021 contract): £63,696 at entry, rising through thresholds to £102,689.
- Consultant (2003 contract): £113,565 at entry, rising to £150,569.
- Specialty registrar pay points: in the circular's table, England values run from £45,715 at the first point to £69,976 at the tenth.
- Scotland, Wales and Northern Ireland set pay separately. Check the relevant national circular if your post is outside England.
Timelines and how to apply, step by step
Plan 12 to 18 months ahead if you need GMC sponsorship and a visa. National rounds open on fixed dates. For the August 2025 Interface Fellowships, JCST announced that applications would open on 18 October 2024 and close on 2 December 2024. The 2026–27 rounds have so far been advertised post by post: a single Hand Surgery post in Edinburgh closed on 9 March 2026, and the Major Trauma round closed on 12 October 2026. The BOA's 2026 travelling fellowship applications closed at 23:59 on 6 July 2026, with online interviews on 3 August 2026.
- 1. Decide which subspecialty and which type of fellowship you want: a JCST Interface post, an RCS-approved SCFS programme, a trust clinical fellow post or a society award.
- 2. Check that you are eligible. UK trainees should check their CCT timing and membership; international applicants should check GMC sponsorship eligibility, recent clinical practice and a valid IELTS or OET certificate.
- 3. Set up NHS Jobs alerts and watch the JCST news page, the Severn PGME recruitment pages and the specialist society fellowship pages.
- 4. Apply with a CV, your logbook and references. Contact the lead trainer beforehand to ask about the operative content and on-call commitment.
- 5. After an offer, international applicants apply for sponsorship (RCS England, RCSEd IPD or another approved sponsor), then for GMC registration, then for the visa using the employer's certificate of sponsorship.
- 6. Do not travel until the GMC has approved your registration. RCS England warns against making travel plans before then. You will then attend a GMC identity check in the UK.

- Read the current recruitment advert for eligibility, start date and pay before you apply.
- Confirm with your sponsor that you meet its criteria and that your IELTS or OET certificate will still be valid when the GMC approves your registration.
- Ask the employing trust for the post's occupation code and pay grade before you apply for the visa.
- Check the GOV.UK visa fee and salary pages again on the day you apply.

Frequently asked questions
Can international surgeons still use the MTI to come to the UK for an orthopaedic fellowship?
Not as a new applicant. NHS Employers confirms that the MTI closed to new applicants from noon on 31 March 2026. Applications already in progress that involved a royal college had a grace period until 30 June 2026. Doctors already on the scheme can complete their posts.
Do I need PLAB for a UK orthopaedic fellowship?
Not if you register through an approved sponsor. The GMC's sponsorship guide does not list a PLAB pass among its eligibility criteria. You need an approved sponsor's Sponsorship Registration Certificate, an acceptable primary medical qualification, and practice in three of the last five years, including ten of the most recent 12 months. RCS England will not sponsor anyone who has failed any part of PLAB.
